Ontario Tech acknowledges the lands and people of the Mississaugas of Scugog Island First Nation.

We are thankful to be welcome on these lands in friendship. The lands we are situated on are covered by the Williams Treaties and are the traditional territory of the Mississaugas, a branch of the greater Anishinaabeg Nation, including Algonquin, Ojibway, Odawa and Pottawatomi. These lands remain home to many Indigenous nations and peoples.

We acknowledge this land out of respect for the Indigenous nations who have cared for Turtle Island, also called North America, from before the arrival of settler peoples until this day. Most importantly, we acknowledge that the history of these lands has been tainted by poor treatment and a lack of friendship with the First Nations who call them home.

This history is something we are all affected by because we are all treaty people in Canada. We all have a shared history to reflect on, and each of us is affected by this history in different ways. Our past defines our present, but if we move forward as friends and allies, then it does not have to define our future.

Incident reporting

An incident is an event that actually or potentially affects the safety of an individual or individuals. This does not include personal injury to the student. Reportable incidents include but are not limited to:

  • medication and treatment errors
  • client falls
  • equipment failures

Within the Nursing Programs incidents are reported and followed up on in accordance with the Policy for Incident Reporting and Recording Profess for Nursing Programs. Please review this policy if you have any questions related to the process.

If an incident should occur:

  1. Ensure the safety of the client
  2. Report the incident to the co-assigned nurse/ preceptor/ clinical sessional instructor
  3. Follow all agency policies, and with support of the co-assigned nurse/preceptor and/or clinical faculty, complete the factual account of the event as per the agency incident reporting process.
  4. Document in the patient’s health record an objective, factual, account of the incident with the assistance of the preceptor and/or clinical sessional instructor.
  5. In consultation with the clinical sessional instructor, co-assigned nurse or preceptor discuss disclosure of the error
  6. Complete the Nursing Program Safety Incident Report Form and submit this to the Nursing Program Office and the year lead/clinical course coordinator
